Travel Documents and Deposit
Each traveler must submit the following to complete their reservation:
- A copy of the front page of their passport (scanned color copy preferred)
- Flight Reservation Form (only complete the basic contact information fields)
- Travel Affidavit
- CBB Travel Information Sheet/Liability Waiver
- $500 deposit/per person (non-refundable) due at time of registration. Deadline is Feb. 27th.

Payments and travel documents should be submitted to the San Francisco Chamber of Commerce, Attn: Cuba Trip, 235 Montgomery Street, 12th Flr., San Francisco CA, 94104. Make the checks payable to the San Francisco Chamber of Commerce. Registration deadline is Monday, Feb. 27, 2012. Final payment is due Friday, March 9, 2012.
